Important information regarding quiz, test and exam accommodations

The Fred Smithers Centre requires at least two (2) weeks notice for quizzes, tests, and midterms.  Two weeks notice means that requests must be submitted fourteen (14) days before the test date, and must be submitted no later than 4:00pm. Due to the volume of requests that we receive, late test requests may not be accepted.

Please use this Test and Exam Accommodation Request Form for all upcoming quizzes, tests and exams.
